As we prepare for more in person gatherings, Coordinating Team is asking all Chalice congregants to fill out our online Chalice Health Covenant.

Filling out this form is voluntary and not required to attend and participate in events at Chalice Unitarian Universalist Congregation. Please be aware, however, that some Chalice gatherings may require full vaccination in order to attend. Proof of vaccination may be required.
